Does the server cache pages?

Because I've changed some content in a php file and it doesn't show up.

Then i tried deleting the file entirely and it i still got the old result - as if it was cached.

Is there a way to purge the cache short of rebooting?

Yes, UMSP aswell as the WDTV itself cache menu content. Just go back to the main menu and the cache is cleared.
If you're asking if there's a cache for file content then no, any changes should be applied instantly.

But I did go back to the menu main, didn't seem to help.

Is there a time factor? Ie, should I wait a while?

Nope. For general debugging look at the logs in /tmp/lighttpd and /tmp/php5.
You can also use the _var_dump_to_file() function which writes to /tmp/umsp-log.txt (e.g. line 15 in control-reply.php)
